ICD-10-CM Code Updates Take Effect October 1, 2026

As we approach October 1, 2026, we want to make you aware of the upcoming FY 2027 ICD-10-CM diagnosis code update.


Each year, the ICD-10-CM code set is updated to reflect changes in medical terminology, clinical conditions, and coding requirements. The FY 2027 update will apply to patient encounters beginning October 1, 2026, and will remain in effect through September 30, 2027. CMS has now released the official FY 2027 ICD-10-CM files and coding guidelines.


What’s changing?

The FY 2027 update includes:

  • New diagnosis codes for certain conditions and diagnoses.

  • Deleted codes that will no longer be valid after September 30, 2026.

  • Changes to existing code descriptions and classifications.

  • More specific coding options in certain areas.

  • Updates to the Official ICD-10-CM Coding Guidelines.

 

What does this mean for your practice?

The biggest thing to keep in mind is that the diagnosis code on the claim needs to accurately reflect what was documented in the patient’s chart. If a code you commonly use has been deleted or changed, it will need to be updated for encounters beginning October 1st. Using an outdated code can lead to claim edits, denials, or the need for corrected claims.


For providers, this is also a good reminder to continue documenting diagnoses as specifically as supported by the patient's clinical situation. If a more specific diagnosis is documented and supported, the appropriate specific code should be reported.


For practice managers, we recommend making sure your EHR diagnosis lists, quick-pick lists, superbills, templates, and any other coding references are updated for the new code set.

 

What should you do before October 1st?

We recommend that practices:

  • Review frequently used diagnosis codes to determine whether any are new, revised, or no longer valid.

  • Update diagnosis quick-pick lists, superbills, templates, and other coding references.

  • Make sure your EHR and practice management/billing systems are updated to the FY 2027 code set.

  • Review documentation requirements for any new or more-specific diagnoses relevant to your specialty.

  • Avoid carrying forward deleted or invalid diagnosis codes on encounters dated October 1, 2026 or later.
